It's marketing now, and <strong>consent rules treat the two differently<\/strong>.\r\n\r\nThe templates themselves live inside whatever tool holds your calendar, and the reminder text is rarely edited where people look first. It's also smaller than a lot of vendor copy makes it sound.\r\n\r\nThe strongest evidence is a meta-analysis of randomized trials. Notified patients had a 15% no-show rate against 21% for patients with no notification, which works out to a 25% relative drop (Robotham et al., BMJ Open, 2016, meta-analysis of 21 randomized studies). Attendance in the same analysis was 67% for notified patients and 54% for the rest.\r\n\r\nKaiser Permanente Washington tried adding a second text for high-risk visits only. High-risk primary care visits saw 7% fewer misses, and high-risk mental health visits in the same program saw 11% fewer (KPWHRI, 2022).\r\n\r\nOne detail people skip: the BMJ Open review found voice calls did better than texts in its secondary analysis. Texting wins on cost and scale, not raw effect.\r\n\r\nMost of this research comes from clinics. A text works on a flip phone. You're billed per segment, so a long text costs double. Business numbers in the U.S. need 10DLC or toll-free registration. Wrong numbers fail silently, which is the one that tends to bite months later.\r\n\r\nEmail gives you room for instructions and add-to-calendar files at almost no cost per send. On the other hand, it ends up in spam folders, gets noticed later, and reports open rates you can't trust.\r\n\r\nThat last problem is Apple's doing. With Mail Privacy Protection turned on, Apple Mail downloads remote content in the background when a message arrives, not when someone reads it (Apple Support). Missed-visit rates were <strong>4.4% with reminders 3 days and 1 day out<\/strong>, 5.8% with a 3-day reminder only and 5.3% with a 1-day reminder only (Steiner et al., AJMC, 2018).\r\n\r\nThe subgroups are more interesting than the headline. For high-risk patients, the extra reminder prevented one missed visit for every 25 people. For everyone else it took 1,328.\r\n\r\nKaiser Permanente Washington came at it from the other direction and sent its second text only to visits flagged as high risk. Taken together, the two programs point to a simple setup. Two automated reminders for everyone is cheap, and it didn't hurt satisfaction in Colorado.
